OFFICE (Port Henry, NY) This is a full time position in our Port Henry Office. It is for weekday, evenings and nights and every other weekend. Care is provided by telephone support and home visits for our patients in Essex County and Northern Warren County. Transportation is required. Mileage is reimbursed. TRI LAKES OFFICE (Saranac Lake, NY) LICENSED PRACTICAL NURSE High Peaks Hospice & Palliative Care
is seeking experienced LPNs. Work independently and as part of our team
providing compassionate care to terminally ill patients in their homes. This is
a per diem position, to work on an hourly basis, often by case or as needed. Hospice office is
located in Saranac Lake. Travel in the Tri-Lakes area is required
