How to Find Us High Peaks Hospice and Palliative Care covers a service area of over 4600 square miles. Providing services within such a broad and in many cases sparsely populated territory can be a daunting endeavor – particularly when you remember that during much of the year we must provide these services in spite of rugged and hazardous winter travel conditions. To better serve you we have three offices.
The Tri-Lakes office, located in Saranac Lake, serves southern Franklin, western Essex, and parts of St Lawrence and Hamilton counties. It also houses the Corporate Offices of the organization. We are conveniently located immediately behind Adirondack Medical Center. North Country
Family Physicians Building The Essex Office, located in Port Henry, serves all of Essex County with the exception of Western Essex county, Which receive services through the Tri-Lakes Office. 4322 Main
Street Warren OfficeThe Warren
Office is conveniently located conveniently located
on Bay Road across from Adirondack Community College.
This, the largest of the three offices, serves all of Warren and parts
of Hamilton counties.
