of the great misconceptions is that only a physician can refer a patient.
Who actually can refer a patient for hospice care?
Anyone.

If
you are not sure whether you or a loved one would qualify for hospice services
contact High Peaks Hospice and Palliative Care and someone will address your questions and concerns.
Criteria for Admission
Hospice
is appropriate for any individual who has been diagnosed with a life limiting
illness and has been given 6 months or less to live. While many hospice
patients are cancer patients we also have patients with a wide variety of other
illnesses including ALS , COPD and Heart Disease.
High Peaks Hospice
and Palliative Care cares for patients of all ages.
We care for infants and children as well as adults.
All too many wait until
the very last moment to ask for hospice care. It is best to contact the
High Peaks Hospice and Palliative Care office
nearest you as soon as a life limiting illness is diagnosed.
Many feel that this is “giving up hope” but rather it is actually giving
hope of a peaceful, comfortable and compassionate death. While you do NOT
have to actually be admitted to High Peaks Hospice and Palliative Care upon initial contact, you and
your loved ones will find that High Peaks Hospice and Palliative Care can offer important, practical
and pertinent advice to help you make educated decisions about end of life care.
Admission
Process
Once you have contacted High Peaks Hospice an informational visit will be
arranged at a time convenient for you and your family. High Peaks Hospice
staff will meet with you to discuss our services and your needs. Your
physician will be contacted to determine if your illness has an anticipated
prognosis of six months or less. If so, you are eligible for hospice care.
