Patient Joan McDermott
wanted to go to the movies to see “My Big Fat Greek Wedding” but her
illness made that difficult. The local movie theater owner
closed the theater one night to everyone but Joan and her caregivers.
They went to the movies and had a marvelous time!
Hospice Services
A team of professionals
and/or volunteers will be available at all times for a High Peaks Hospice
and Palliative Care
patient. The team’s goal is to assist in enhancing the quality of life
of the patient and their loved ones as well as bringing physical and
emotional care.
The “team” is a dedicated group of nurses (RNs
and LPNs), social workers, home health aides, a hospice physician,
bereavement counselors, trained volunteers and nutrition counselors and
therapists. The extended team also includes the patient’s own
personal physician, family, friends and neighbors. Whatever the
setting, the team works closely with the patients and their families to
plan care that is safe, comfortable and the choice of each patient.
Once an individual has been admitted into hospice
care the following services are provided to the patient and their
families:
Most medication related to Hospice diagnosis
Equipment such as wheel chairs, hospital beds, oxygen and other medical
equipment necessary to make the patient comfortable
Registered Nurses available 24 hours a day should emergency needs or
questions arise.
Licensed Practical Nurses or Home Health Aides for personal care
Social Worker for in home counseling with patients and loved ones
Bereavement Counseling & Spiritual Care
Therapists – Occupational, Physical or Nutritional
Respite Care
Volunteer for patient socialization and caregiver respite
What are not listed here are the intangibles.
Once admitted the day to day stress of the caregivers is eased – hospice
staff and volunteers are there – as often or infrequently as you wish
– to help with day to day tasks and to provide comfort and care to not
only the patient but to their families and loved ones as well.
Prescriptions are refilled, equipment is found and delivered to wherever
the patient is and High Peaks Hospice and Palliative Care works with insurance companies to
alleviate the family of this burden. Much of the pressure is
lifted enabling the patients to live their lives to the fullest until the
very end.
