At the beginning of life we are surrounded by comfort and care, at home, don’t we deserve the same at the end?
Nancy was able to look out on her beautiful garden the morning she dies because hospice was there.



 

  • When polled, 67% of Americans say that if they had a choice they would like to die at home. 

  • Unfortunately, in 2000 of those who died in New York only 16% actually died at home.

  • While hospice care can be provided anywhere that the patient chooses – the majority of High Peaks Hospice and Palliative Care patients are able to die in the place they feel most comfortable surrounded by those they love and who love them with support from the hospice team.


Where does High Peaks Hospice and Palliative Care provide care?

HOME
Hospice care provided in the home of the patient or caregiver enables the patient to remain in the care of loved ones in the place that is most familiar to them and gives them the most comfort.
 
HOSPITAL                 
Short-term care for control of symptoms that temporarily cannot be managed at home is provided in local hospitals affiliated with High Peaks Hospice.
 
NURSING HOME
Hospice staff work with and support Nursing Home staff to enhance the quality of life of the Nursing Home Resident.
 
OTHER LOCALES
Hospice is willing to arrange care in any setting that wishes to collaborate with hospice staff in enhancing the quality of life of the patient.
